Sports Medicine Packet

All sports medicine packets are due by AUGUST 1, 2012 REGARDLESS OF WHEN YOUR SPORT'S SEASON BEGINS.

Instructions for completing the sports medicine packet:

1. Please download the appropriate packet and complete all forms.  NOTE: There are two different packets this year - one for new student-athletes and transfers and one for returning student-athletes.

2. Please use an ink pen when completing all forms.
